We Deliver Five Diverse Types of Support
Intensive Intervention Support
Support Facilitators work alongside Families in their homes and within the community. Support is tailored to minimise longer term Statutory Interventions this could include assisting reunifications, supporting Parental Agreements, or wrapping supports around Kin and Foster Placements.
Weekend Respite and Emergency Response
Support Facilitators can provide planned weekend overnight or day support respite to support placements and Young People under pressure or in distress.
Transportation Assistance and Supervised Contacts
Support Facilitators can facilitate supervised family contact this may also include transport to and from locations. Support Facilitators can provide basic transport support to alleviate pressure from placements.
Residential Placement Support and Independent Living Skills
Support Facilitators assist Young People who are struggling within residential placements, requiring assistance to transition to independent living models or stabilise in care homes.
Youth Engagement: Outreach, Mentoring and Diversionary Support for Young People
Support Facilitators assist Young People who are disengaged from their placements and schooling, at risk of homelessness, and under Youth Justice orders. Support Facilitators 'go to' the Young Person; meeting with them to ensure general welfare and wellbeing is maintained. Support Facilitators can link these clients with any necessary networks such as Centrelink, doctors, counselling, work experience items, and can transport to any necessary appointment (Youth Justice, job seeking, medical, etc). Support Facilitators engage in diversionary and activity based-therapy strengths-based approach to encourage Young People to succeed in their endeavours and redirect to more positive life choices and outcomes.
Young People
We provide an outreach to support Young People at risk of homelessness, transience, criminalised behaviours, and who require assistance in transitioning between homes or placements while assisting to develop independent living skills.
Families and Carers
We provide In Home Support to assist parents (Biological, Foster & Kin) to sustain family units, and assist in the prevention of statutory interventions.
Service Providers
We work alongside Service Providers to offer additional support for complex cases. This includes client care planning and case management, staff training, case consultation, panel review, and transition planning.
In Home Support
Support Facilitators work alongside families in their homes across a variety of days and times to best suit the needs of the family. This can include early mornings, evenings and weekends.
Assisting Reunifications
Support Facilitator's work alongside parents and where required carers to assist all parties throughout the reunification process.
After Hours Supports
Parents, Carers and Young People are able to access after hours supports to assist in crisis containment when face to face sessions are not occurring.
Client-Centred Plans
Support Facilitators work collaboratively with parents and stakeholders to ensure all plans and supports are tailored for each Young Person.
Skill Building for Increased Stability
Support Facilitator's role model therapeutic strategies and provide coaching alongside parents to assist in embedding new skills.
Build on Community Supports
Support Facilitators assist Families to build on their current support networks within the community to provide stability in the home.
Weekend Overnight Respite
Providing regular or emergent overnight respite options to sustain long term placement options.
Day Support Respite
These supports include after hours and weekends wherein Young People can engage in activities outside of the placement and develop regulation strategies.
Empowering Young People and Carers
Regular respite provides Carers and Young People time for self-care, de-escalation of high stress situations and preservation of relationships.
Transportation Assistance and Supervised Contacts
Support Facilitators can assist in supervising contact arrangements in the community, family homes or contact centres. Detailed reports are able to be provided along with transport to and from contacts.
Transport Support
Support Facilitators can provide basic transport assistance for clients to appointments, including practical support to and from school, supervised contact arrangements or longer distance travel requirements.
Supporting Placement Stability
Transport support or Supervised Contact support often relieves pressure from placements and carers, assisting with longer term placement stability.
Stability for Placements
Support Facilitators can assist in supervising contact arrangements in the community, family homes or contact centres. Detailed reports are able to be provided along with transport to and from contacts.
Support Transitioning to Independent Living
Assisting Young People to develop their independent living skills and wrapping supports around until their capacity builds.
Hands on Approach
Support Facilitator's can provide 1:1 supports to assist Young People in developing more effective coping strategies during times of stress or crisis. Group sessions can also be facilitated to assist Young People in managing varying relationship dynamics within the household.
Collaboration
Support Facilitators work alongside existing supports, creating a collaborative group of service professionals to wrap around the Young Person holistically.
Emergency Overnight Care
Providing emergent overnight support when a placement or relationship breakdown has occurred.
Assist Disengaged Young People
Support Facilitators assist Young People who are disengaged from their placements and schooling, at risk of homelessness, and under Youth Justice orders.
"Go to" Approach
Support Facilitators ‘go to’ the Young Person; meeting with them in the community or at known frequented locations to ensure general welfare and wellbeing is maintained.
Safety and Containment
Supporting Young People who are choosing to self place by ensuring they have access to regular supports and basic necessities, assistance to implement safety plans should crisis occur including who they can reach out to.
Link Young People with Services
Support Facilitators can link these Young People with any necessary networks such as Centrelink, doctors, counselling, work experience items, and can transport to any necessary appointments (Youth Justice, job seeking, medical etc.).
Mentor Young People to Success
Support Facilitators engage in diversionary and activity based-therapy, role-modelling and mentoring, using a strengths-based approach to encourage Young People to succeed in their endeavours and redirect to more positive life choices and outcomes.
After Hours Support
Young People are able to access after hours supports to assist in crisis containment when face to face sessions are not occurring. This can also ensure regular contact is maintained when Young People are transient.
